this appointment is for Review the current medication, dosage, and timing. Monitor symptom response, including attention, hyperactivity, impulsivity, and emotional regulation. Assess for potential side effects and tolerability. Record physical observations such as blood pressure, pulse, and weight. Review any relevant medical history, co-existing conditions, and concurrent medication use. Provide psychoeducation around medication, including expected benefits, possible side effects, and safe use. Adjust the medication dose or formulation as clinically indicated, in line with national guidelines. Agree a clear follow-up plan, including ongoing monitoring and communication with the wider healthcare team where appropriate. The aim of titration is to identify the most effective medication and dosage for the individual, balancing symptom improvement with minimal side effects. This process is collaborative, with the patient actively involved in decision-making throughout. Once medication has been titrated and maintained at an appropriate therapeutic dose, I will arrange for shared care to be transferred back to the patient’s GP for ongoing prescribing and monitoring, ensuring continuity of care within primary services.
