ADHD Medication Titration – Face to Face This follow-up appointment is for patients currently receiving ADHD medication through our service who require a review of treatment response, side effects, tolerability or further dose adjustment. During the appointment, we will: • Review the current medication, dose and timing • Monitor symptom response, including attention, hyperactivity, impulsivity and emotional regulation • Assess possible side effects and tolerability • Record physical observations, including blood pressure, pulse and weight • Consider relevant medical history, co-existing conditions and other medication • Provide information about expected benefits, possible side effects, safe use and monitoring requirements • Adjust the medication dose or formulation where clinically indicated and safe • Agree a clear follow-up and monitoring plan Medication changes and prescriptions are subject to clinical assessment, appropriate physical-health monitoring and safe prescribing requirements. A dose change or prescription cannot be guaranteed where required information is missing or there are clinical safety concerns. The aim of titration is to identify an effective and well-tolerated medication and dose. This is a collaborative process, with the patient involved in decision-making throughout. Once medication is stable at an effective and tolerated dose, we can submit a shared-care request to the patient’s GP for ongoing prescribing and monitoring. Acceptance is entirely at the GP practice’s discretion and cannot be guaranteed. If shared care is declined, we will explain the available arrangements for continuing private prescribing and monitoring, including any applicable fees.
